Investigation Summary
On May 30, 2007, an employee, a forklift driver was working the night shift and had agreed to meet a coworker near the loading docks on the north east corner of the facility for a break. The employee was driving an order-picker type industrial truck manufactured by Crown Forklift, Serial Number 1A131760. The truck had a sticker on its mast indicating a shop designation "10." At the time of the accident, the employee drove the truck onto the north east loading docks, apparently swerved and drove off the dock falling a distance of 4 feet 5 inches to pavement below. The employee was hospitalized and treated for liver injuries.
