Investigation Summary
At approximately 4:00 p.m. on July 26, 2007, Employee #1 was performing a regularly assigned task of sweeping the lot in the attempt to keep the area clean. Employee #1 was assigned to continually sweep the lot with a push broom. While sweeping the lot, Employee #1 was passing behind a forklift that was actively unloading steel bins from a semi-trailer. The forklift operator began to travel in reverse, striking Employee #1 on the right side of his body, resulting in a compound fracture of his right lower leg. Employee #1 was hospitalized.
