Investigation Summary
At approximately 8:00 p.m. in July 17, 2010, an employee was assigned to work in the printing department with his supervisor and another machine operator on the employer's Bestech 40 Akiyama printing machine. The Bestech 40 Akiyama printing machine is approximately 40 feet in length and has 6 in-line printing units. Computer Controls are on the north end of the machine and the south end. Each unit is equipped with emergency shut-off switches. Prior to the accident, the Supervisor was at the north end of the machine watching the finish prints exit the machine, while another operator was at the south end of the machine, loading the machine with blank printing media. The employee stated he was working along side the Supervisor, when the supervisor noticed a smudge or imprint on one of the final prints. He told the employee to check the printers. The accident occurred while the employee was using a cleaning technique taught to him by his supervisor, which is to spit on the roller and let the saliva self-clean the roller. But when he spat on the roller, some of the saliva landed on a bar, in close proximity to the in-running rollers of the printing machine. As he attempted to clean the spit from the bar with a hand held rag, the rag was caught by the in-running rollers and pulled the employees right hand into the in-running rollers. The employee's right middle finger was crushed by the rollers and needed to be surgically amputated at the second knuckle. After the accident, the employee was driven to San Miguel Clinic by a coworker but the clinic was closed. A police patrol car was driving by and gave assistance to the employee by calling the Fire Department and the Paramedics. The employee was stabilized then driven to Los Angeles County USC Medical Center and treated for the crushing injury to his right hand. The employee remained in the hospital for approximately 15 days.
