Employee Sustains Contusions and Abrasions in Fall

On April 19, 2007, Employee #1 was working as a construction laborer for HBParkco, an incorporated company that was engaged in structural concrete erection at a building. Employee #1 was installing spray foam around the east side of a column on the ground level deck of Building D at the jobsite, when he fell approximately 12 feet from a deck to a concrete slab below. He had been equipped with a personal fall arrest system, but he had not tied it off at the time of the accident. He sustained bruises, contusions, and abrasions to his back. He was hospitalized. Cal/OSHA determined that the accident was caused by Employee #1's failure to tie off the fall arrest system, when exposed to a potential fall hazard while working around openings.
