Investigation Summary

At approximately 4:00 p.m. on April 17, 2007, Employee #1 was installing a television satellite dish at a private resident. The dish was to be mounted on the edge of the roof approximately 17.5 ft above a concrete driveway. In order to access the roof edge the employee placed a Werner 28-foot fiberglass and aluminum extension ladder on the driveway against the house at ratio of less than a 4 to 1. After installing the dish, the employee entered the residence to determine if the customer was receiving a strong signal. After determining that the signal was not of proper strength, the employee climbed back up on to the ladder to realign the dish. When he reached the dish, he began to remove a wrench to loosen the dish mount when the feet of the ladder slipped out from beneath him, causing him to fall to the concrete. Employee #1 was injured (not specified) and hospitalized. The slope of the driveway was approximately four degrees, and there was oil present on the concrete at the time of the incident. The employee stated that he did not place the ladder on the oil, and the oil did not appear to be disturbed at the time of inspection. Cal/OSHA determined that the accident was caused by the extension ladder slipping out at the base. No accident-related citations were issued to the employer.
