Investigation Summary
On March 20, 2007, Employee #1, a laborer, was stenciling numbers on large blocks of foam. He gained access to an elevated conveyor via a short, fixed steel stairway equipped with proper handrails and a top platform. The height of the platform's top was in line with, and at the same level as, the top of the conveyor frame. Employee #1 stated that he was standing on the top of the conveyor and that he had been working for the company for nine years doing this sort of thing, and had never had a problem before. According to him, the conveyor started and stopped automatically and there was an audible buzzer that sounded when the conveyor was going to start. He heard a buzzer, as was usual, but upon turning he lost his balance when the conveyor started. He fell 3 1/2 feet from the top of the conveyor to the floor. He sustained serious injury, including a fracture of his left leg, which required his hospitalization for more than 24 hours.
