Investigation Summary
On February 20, 2001, Employee #1 was cutting metal bands that were wrapped around a stack of bars that were on a pallet that was supported on the forks of a forklift truck. When Employee #1 was standing in front of the load when he cut the metal bands, one of the steel bars weighing about 750 lbs. fell from the stack of bars, striking Employee #1. The load was not adequately secured on the load-engaging device and the pallet holding the metal bars was not adequately supported by the forks of the truck. Employee #1 was experienced in the job being performed. Employee #1 was driving an industrial forklift truck manufactured by Hyster, serial number C5D 12647 S, but had not been trained or certified by the employer to operate the vehicle. Employee #1 was hospitalized and treated for his injuries.
