Employee Caught Between Shelf and Lift, Amputates Foot

At approximately 7:00 a.m. on November 25, 2007, Employee #1 was using powered industrial vehicle to pull items from the shelving for individual customer orders. Employee #1 had been pulling orders and as he was turning a corner in the warehouse, the reach truck bumped the protective bumper of the shelving which jarred him, catching his left foot between the shelving bumper and the reach truck. Employee #1 suffered the amputation of his foot and was hospitalized.
