Employee's Fingertip Is Amputated in Punch Press

At approximately 6:55 a.m. on December 7, 2007, Employee #1 was operating a full revolution, punch press, Bliss, Model J Safeguard, and he was making metal parts. When Employee #1 started work at 6:30 a.m., he was set in pull backs by a coworker. Within a half an hour, Employee #1 left his work area. Upon his return to his workstation, Employee #1 did not put his straps back on and started operating the machine. Within minutes, Employee #1's fingers on his right hand were smashed. The tip of his right middle finger was amputated. Employee #1 was hospitalized. The company provided overall training to the employees before they were assigned to work. One of the agenda items stated that "if your press includes pull back restraints, you are required to have them on in the correct manner at all times, while operating or working within the press area." Employee #1 stated that he did not remember removing his pull backs, going to visit another worker, and then returning to his workstation. A coworker, who heard Employee #1's yell for help, stated that Employee #1 was holding his right arm with his left arm, and he was bleeding. However, there was no blood on the wristlets. Reports obtained from employees at the facility stated that safety was always stressed and that employees were told to wear their pull back restraints at all times.
