Investigation Summary
At approximately 4:00 p.m. on November 26, 2007, Employee #1, of Delta Heating and Cooling, Inc., was ascending an extension ladder to a mezzanine approximately 12 ft above a dry concrete floor. The floor had dried mud on it from coworkers' shoes. Employee #1 was climbing the ladder when he fell approximately 9 ft to the floor. He sustained serious injuries and was transported to the hospital, where he died on November 28, 2007. Three coworkers were present at the site, but did not see the accident. The ladder was not extended 3 ft above the upper landing, nor was it secured at the top. The employer did not have a competent person perform regular inspections of the work site. The employer had not trained Employee #1 in the safe use of a ladder, although he did attend a safety meeting given by the general contractor at which he was reminded to secure ladders.
