Employee Is Crushed and Killed by Boom Arm

At approximately 1:15 p.m. on July 30, 2007, Employee #1 had been instructed to drain the machine fluids in order to get the loader to operate. This would have required Employee #1 to cycle the machine in order to remove water from the lines. The bucket attachment was still connected to the machine. The safety boom locks were not utilized. When Employee #1 was found, hydraulic oil covered his face and the upper half of his body and he was found pinned face down by the boom arm, with the bucket attached, across the front end of the skid steer. All employees had attended training on the skid steer loader equipment that was provided by the manufacturer. Employee #1's abdomen and lower back area were crushed. Employee #1 was killed.
