Employee Dies from Injuries When He Falls Off Roof

At approximately 8:15 a.m. on April 18, 2007, Employee #1 was placing insulation washers on a flat roof at a construction project for a restaurant. He was walking backwards, towards the unprotected edge placing the washers. Employee #1 caught his heel or slipped on the 1-ft parapet at the edge. He fell off of the roof and landed on his head. Employee #1 was not wearing fall protection. He was transported to the hospital, where he died three days later.
