Employee Is Killed in Fall from Elevation

On April 17, 2008, an employee was constructing a work platform on the underside of a highway overpass, using grade 2 or better pine 2-in. by 12-in. by 7-ft 1-in. boards. He fell through the platform after putting his weight onto a knotted board that gave way and broke under his weight, causing him to fall. The employee was killed. The employee had been equipped with personal fall arrest system and was observed entering the work area wearing the harness. For some unknown reason, the employee had removed his harness and was unprotected at the time of the accident. The harness was found lying on the platform being constructed.
