Investigation Summary
On July 10, 2007, Employee #1 and a coworker were installing a copper roof on several towers of a 7,000 sq ft home. Employee #1 was apparently accessing the roof from a job-made ladder when he fell 12 ft onto a rock walkway and was killed. He was not wearing a harness, nor was any other form of fall protection being used. There were no witnesses to the accident.
