Investigation Summary
On October 18, 2006, an employee, a truck driver, had just delivered a batch of material at the construction site and was waiting for a load of waste material to be loaded into the end Dump Number 232. While waiting for the material to be loaded, the employee exited the cab and began walking across the yard toward another vehicle that was approximately 45 feet away. Meanwhile the driver of a front end loader, a coworker, who had been loading the end dump with waste material had just dumped a load into the end dump and was reversing the loader when loud shouts and screams alerted her to stop. The coworker exited the loader, where she witnessed the employee laying under the back wheel of the loader. The Emergency assistance was subsequently called, and the employee was transported to the hospital to address her wounds. The employee was hospitalized and treated for fractured arm.
