Investigation Summary

Investigation Nr: 202461844
Event: 12/22/2006
Employee Dies after Fall from Roof of House

Employee #1 was wrapping a residential house under construction in Tyvek. When he came around to the front of the house, he used the roof section to walk on as he installed the paper. At that point, the superintendent drove onto the site and saw Employee #1 on the roof. The superintendent yelled to Employee #1 and told him to get down from the roof. As the superintendant turned his attention to other work, Employee #1 fell from the roof. He was transported by emergency services to Englewood Hospital where he later died. Further investigation revealed that the employee was not supposed to be on the roof, was only supposed to be doing light clean up work, and was not wearing a harness and lanyard for fall protection.
