Investigation Summary
At approximately 1:00 p.m. on October 17, 2007, Employee #1, a field operations manager, was working in one of the employer's rice seed drying storage locations in Williams, California. There were five corrugated metal silos approximately 14 ft 5 in. tall in this location. Employee #1 was opening the vents at the top of the silos. He was jumping from roof to roof, rather than getting down and moving the ladder. Four of the five silos were situated approximately 8 in. from roof to roof. Employee #1 jumped between them without problem. Employee #1 misjudged the distance on the last jump, which was approximately 36 in. from roof to roof, and fell between the last two silos. A coworker, who was on his way to the site to assist Employee #1 found him on the ground conscious, when he arrived. Employee #1 fractured his forearm and was hospitalized. According to all accounts, the company does not regularly open or close these vents and this was not a typical procedure for opening the vents. A ladder was provided and employees were expected to use the ladder to get access to and from the roof tops. Employee #1 was working alone when the accident occurred.
