Investigation Summary

Investigation Nr: 362335333
Event: 09/27/2007
Employee Is Injured in Fall from Slippery Roof

On September 27, 2007, Employee #1 arrived at the worksite before the sun had risen high enough in the sky to thoroughly melt all the frost from the roof where he was about to stack materials. The roof had a pitch of 7 and 12 at the higher end and 3 and 12 at the lower end. The edge of the roof was 12.3 ft above the platform below. Employee #1 was moving around the 7 and 12 portion of the roof sweeping the frost when he stepped on a slick spot. He lost his footing and fell off the roof. He suffered serious injuries to his neck, forehead, back, and left eye socket and was hospitalized.
