Investigation Summary
On August 15, 2007, a lead custodian fell while reaching to install ceiling tiles. The fall from the step ladder was 4 feet to the floor. The employee sustained concussion, ear drum, and clavicle injuries. The employee was hospitalized. The ladder type and condition reported indicated that the equipment and procedure were not in violation. The possible explanation from the reports of witnesses was the rung was overstepped or the lean of the employee was too far off balance.
