Employee Fractures Ankle in Fall from Ladder

On May 10, 2007, an employee was assigned by the Safety Director to check the level of the container of acid used to clean the stainless steel pipes. The Safety Director did not specify how or what equipment the employee should use. The employee selected the 12-ft A-frame fiberglass ladder, leaning it on the cylindrical container, and climbed to approximately the third rung from the top and was looking into the container, when the ladder slid out from underneath him. He fell from a 12-ft ladder fracturing his left ankle. The employee was hospitalized and treated for a fractured ankle.
