Investigation Summary

Investigation Nr: 202485553
Event: 05/22/2007
Employee's Fingers Are Lacerated in Machine

On May 22, Employee #1, a packaging line worker, was assigned to the company's 5-10 pound bag packaging line. He detected problems with the seal of the bags. He determined that paper tabs that are cut off during the sealing process had jammed in the discharge area. He did not turn off the packaging line and lifted up a guard to expose the compartment where the cutter-blower assembly was located. He reached inside and his thumb contacted a moving chain and sprocket drive. His first three fingers of his right hand became entangled in the chain and sprocket drive before he was able to shut the system off. The chain was cut to remove his hand and he sustained severe lacerations to three fingers, receiving stitches.
