Employee Is Injured When Struck by Platform

On September 26, 2006, Employee #1 was struck by a falling lift platform that had been supported by the forks of a forklift after he removed both maintenance safety chocks. He was hospitalized for a crushed right lower leg and foot injuries, as well as collar bone and spine injuries sustained while escaping from under platform.
