Investigation Summary
On June 11, 2007, Employee #1, a truck driver, was working alone in the truck's freezer. With the ignition switch still on, Employee #1 climbed up a forklift inside the freezer and fell about eight feet. As he fell, one of his feet came in contact with the forklift tilt control lever. This caused the forklift mast to tilt toward the forklift body, and Employee #1's stomach became caught between the forklift over head guard and the mast tie bar. Employee #1 was killed due to his injuries.
