Employee Is Injured When Struck by Makeshift Hook

On August 21, 2006, Employee #1 was using a makeshift hook to fasten the universal yoke of a rear-end differential from a heavy duty commercial truck to the safety latch hooks of a two-branch alloy steel chain sling. When the makeshift hook failed, it struck his face and mouth, causing multiple tooth and facial fractures.
