Employee Is Injured When Grinding Wheel Breaks Apart

On August 3, 2006, Employee #1 was operating a swing frame grinder, while grinding a tool. The abrasive wheel broke apart, and he was struck by a piece of the wheel. He was hospitalized for a laceration to the top of his head.
