Employee's Toe Is Amputated in a Forklift Accident

At approximately 6:45 a.m. on July 11, 2006, Employee #1, a forklift operator, was removing a pallet of raw materials from the back of a semi-tractor/trailer. As Employee #1 entered the back of the trailer with the forklift, the semi-tractor/trailer pulled away from the loading dock. Employee #1 tried to jump off the forklift back onto the loading dock, when the forklift fell from the back of the semi-tractor/trailer onto his left foot. Employee #1's foot was caught between the back of the forklift and loading dock, partially amputating the big toe on his left foot. The forklift fell into the loading dock pit and sustained damage to the cab.
