Employee Is Killed When Load from Forklift Falls

On February 13, 2007, an employee was told to unload coolers from shipping crate. No one was available to assist him. After unloading the first crate, the employee used the forklift to drag the cooler from the shipping container and then placed 4-ft by 4-ft board under one end to stabilize the load with the rear end resting on the container while he drove around to the other side to pick it up. When he went to lower the load the 4-ft by 4-ft board became wedged. The employee got off the forklift and tried to knock the board out. When he did, the load flipped over on top of him, crushing and killing him.
