Investigation Summary
On September 18, 2006, an employee was operating a three wheeled sand bunker rake. He was performing routine work of grooming the sand bunkers with the rake attached to the three wheeled vehicle. The employee was raking the bunker at Hole Number 6 of the ridge section. The putting green sits on a plateau and there were two bunkers on the west side of it on a grade. The employee was leaving the front sand bunker, with a 20 degree slope, while riding the vehicle and going to the back one that was approximately 6 ft away. Upon exiting the bunker, the vehicle flipped over on the grade. The vehicle flipped over on top of the employee then rolled off and landed upright on its wheels approximately 36 ft from the bunker. The slope of grade within 6 ft of the bunker was 30 degrees. The manufacturer prohibited operating the vehicle on any grades over 20 degrees. The employee was transported to the maintenance shop approximately one mile away, by other coworkers via a golf cart. He was administered first aide and airlifted to the county hospital. The employee suffered a compound fracture of his right forearm requiring hospitalization and surgery.
