Employee Is Killed When Fork Lift Tips Over

On August 2, 2006, Employee #1 was in a personnel platform attached to a telescopic handler forklift. He was elevated approximately 42 ft. The forklift tipped over, causing the Employee #1 to fall. He was killed.
