Investigation Summary

Investigation Nr: 202550620
Event: 03/28/2009
Employee Is Injured When Struck by Falling Load

On March 28, 2009, Employee #1, a foreman, was managing a framing job for a building under construction. While Employee #1 was walking the job site, two coworkers were handling materials to be used for framing. One coworker was operating a gradall to place material on the roof of the building, while another coworker served as a spotter to signal when it was safe to lower the load. The drop off was to be located over a parapet, which required such additional precautions as using a spotter. As the gradall lowered a load, measuring 2 inches by 4 inches by 4 feet, the truss holding the portion of roof to contain the materials broke in half, causing the truss and the entire load to fall approximately 30 feet. Employee #1, who was walking under the sheathed roof, was struck by the load and knocked unconscious. Employee #1 was taken to a nearby hospital, where he was hospitalized and treated for multiple unspecified injuries to his face and body.
