Employee's Finger Is Amputated by Gate Assembly

On November 25, 2008, Employee #1, an assembly mechanic employed full-time by an oil-drilling equipment manufacturer, was working in the manufacturing assembly area. Employee #1 had just completed installing a gate assembly onto the main body of a clamp cylinder body. He had forgotten to install two spring bolts, and he had already taken off the rigging strap from the crane and gate assembly. Employee #1 removed one pin from the gate assembly and pivoted open the gate from the rest of the assembly to install the parts. Once opened, the entire assembly became unstable, and as the assembly fell, the gate closed and caught Employee #1's finger between the gate and main assembly. Employee #1's right ring finger was amputated at the middle section of his finger tip. Employee #1 was transported to UCI (apparently university of California Irvine Medical Center) in Orange, CA, where he was treated for his injuries and underwent surgery. The Division found no serious, accident-related violations.
