Employee Steps Into Floor Opening and Fractures Ankle

On November 3, 2008, Employee #1, who was constructing a stage extension in the theater for a play, fractured his right ankle when he inadvertently stepped into the gap on the platform. The platform was approximately 2 feet 6 inches high. While installing the stage it was necessary to cover a gap (approximately 8 to 10 inches wide) with sheets of plywood, Employee #1 was measuring the plywood to fit; when inadvertently stepped into the gap and fractured his right ankle. Employee #1 was transported to Kaiser Permanente, where he was treated for his injuries and referred to Orthopedic for surgery.
