Employee Is Crushed When Mixer Tips Over

On November 25, 2006, Employee #1 was assisting with moving equipment from one plant to another. He was inside a truck trailer. A coworker was operating a forklift, and was unloading a mixer from the trailer when the mixer began to tip. Employee #1 tried to prevent the mixer from tipping but was killed when he was crushed between the mixer and the side wall of the trailer.
