Investigation Summary
On August 22, 2006, a trained lift truck operator was returning a SkyTrac B8400 lift truck to a fenced equipment storage area. He stopped to speak to Employee #1, the company superintendent. After their conversation, the operator looked to see if Employee #1 was clear before driving off. When another company employee yelled to indicate that Employee #1 had been struck, the operator stopped. Employee #1 was hospitalized for unspecified fractures.
