Investigation Summary

At approximately 1:30 p.m. on October 9, 2006, Employee #2 was installing a skylight in a third floor ceiling, about 32 ft in elevation. This was above an open stairwell. Employees #1 and #2 placed two, 14-ft long by 9.5-in. planks across the stairwell, and then made a makeshift ladder by nailing 1-in. batons across the planks approximately one foot apart. They positioned the makeshift ladder on the two planks, and placed the top into the skylight. Employee #1 was climbing the ladder, however, his foot slipped off a baton, and he fell approximately 20 ft to the entryway of the stairwell. He sustained fractures to his right leg and back.
