Investigation Summary
At approximately 10:00 a.m. on or about August 16, 2006, Employee #1 was operating a 2000-pound forklift. He used the forklift to carry pallets of scrap paper to the dumpster. On his approximately sixth trip, he was returning to the warehouse with the forks raised two feet in the air carrying no pallet. He spun the forklift around on a 5-degree down slope to return to get something. The forklift tipped over. The employee tried to jump out, but the fall over protection caught him across the abdomen. Coworkers lifted the forklift off of him and he was transported to a local hospital. Employee #1 sustained fractures to his ribs, sternum and vertebrae and was hospitalized.
