Investigation Summary
On February 14, 2007, an employee was performing manual turn-up on an in-line coated paper machine at the reel to reel drum area. A coworker activated the lowering of the primary arm from the machine operator's controls and did not see the employee. The employee was killed when the primary arm of the machine was lowered and crushed the employee between the primary arm and stationary secondary arm rail. The machine was not locked or tagged out.
