Investigation Summary
At approximately 1:15 p.m. on December 27, 2006, Employee #1 was working on the eave of a roof, 27 feet high. Due to inclement weather, all of the employees had worked intermittently for three to four days, removing old plastic sheeting and skylights on the roof of a warehouse, and replacing them with new plastic sheeting. Employee #1, who had arrived for work 45 minutes prior to his foreman, fell through one of the old plastic skylights, and died as a result of the fall.
