Employee Is Killed in Fall from Elevation

At approximately 6:00 p.m. on September 6, 2006, Employee #1 was working from a platform held aloft by a rough terrain powered industrial truck. Employee #1 was installing hardy plank siding at a height of approximately 15 ft along the exterior wall of an apartment complex. The work platform was not secured to the industrial truck and Employee #1 was not wearing personal fall protection or a restraint device. The work platform, along with Employee #1, fell from the industrial truck. Employee #1 suffered head injuries from his fall to the concrete parking lot and died at the scene.
