Employee Is Killed When Struck by Falling Pallet

At approximately 9:00 a.m. on August 28, 2006, Employee #1 was operating a forklift at the southeast side of Building Number 5. The storage rack bays were approximately 12 ft wide and 7 ft high. The racks were four bays high, for a total of 28 ft in height. Each bay contained 3 pallets with 2500 lbs of sugar. The top level held 3 pallets with lightweight boxes. When the forklift struck the rack, 6 pallets loaded with 50 lb bags of sugar fell. Employee #1 was struck by falling pallets and was found dead less than 4 ft from the forklift.
