Employee Is Killed in Fall from Elevated Platform

On September 26, 2006, Employee #1 was on an elevated job-made work platform which had been placed unsecured on the folks of an all-terrain folk lift. He was maneuvering a 97-lb glass window pane through a third-story unframed window opening. When the platform shifted, Employee #1, five window panes, and the platform fell 23 ft. After landing on the ground, Employee #1 was struck by the glass window panes, which shattered and caused multiple lacerations to his head and upper body. Employee #1 was killed.
