Employee Falls from Ladder and Is Killed

At approximately 4:55 p.m. on February 23, 2007, Employee #1 was painting the outside window trim of an existing residential home from a portable ladder. Employee #1 carried a gallon container of paint while ascending and descending the ladder. The ladder was not secured to prevent displacement and was placed in a passageway. The company's records indicate that a training program was not provided to employees who used portable extension ladders. Employee #1 fell approximately 15 ft to ground level and the ladder fell with him. Employee #1 sustained an unspecified fracture and was killed.
