Investigation Summary
At approximately 12:15 p.m. on June 22, 2007, Employee #1, a laborer who did not speak or understand English, was installing concrete tiles on the second section of a roof at a new residential construction site. Two coworkers were also at the same level of the roof, conducting the same type of work. Employee #1 was killed when he fell approximately 10 ft onto the roof of a vehicle garage and then fell another 10 ft to the ground. At the time of the accident, Employee #1 was performing a routine task that was frequently conducted by the laborers for approximately 8 hours per day. It was later determined that all three employees lacked training in recognizing fall related hazards. They had not been instructed in safety related subjects such as, but not limited to, the nature of fall hazards in the work area, safety monitoring systems, controlled access zones, the role of fall protection plans, and procedures to minimize fall hazards. The company did not have a fall protection plan in place that required the use of fall protection for heights over 6 ft, nor was an alternative fall protection plan in place that met the requirements of 29 CFR 1926.502(k).
