Investigation Summary

Investigation Nr: 202466710
Event: 05/06/2008
Employee Falls While Servicing Conveyor

A triangular-shaped diverter that hooked onto a slot on the metal sidewall of a belt-conveyor system came loose. A supervisor called the control tower on his radio to stop the conveyor belt system so that he could replace the diverter. After the conveyor belt stopped, he replaced the diverter. The conveyor belt started moving before he got down from the conveyor. The movement caused him to fall from the conveyor. He fell 1.2 meters from the side of the belt onto an operator-station landing. He was hospitalized with a fractured leg.
