Investigation Summary
At 9:30 a.m. on May 7, 2008, Employee #1, a service technician, suffered serious head injury due to a fall of 15 feet through an unprotected skylight. Employee #1 had been working for a medium sized HVAC service company for 8 months total over a 3 year period. He was performing the duties of a HVAC Service Technician, including ascending roofs to repair, replace, or maintain HVAC units. At the time of the accident, he was performing a regularly assigned task of performing scheduled maintenance to HVAC units on the roofs of two buildings for the onsite employer. Employee #1 was assigned a two-day task of servicing the HVAC units. Employee #1 had commenced servicing the first of multiple HVAC units and was attempting to test the unit when Employee #1 fell through a skylight on the roof of the structure. Employee #1 fell 15 feet, striking the concrete below. Employee #1 was working alone at the time of the incident and was not protected from falling through the skylight. Employee #1 was transported to USC Medical Center where he was treated and hospitalized for 3 weeks.
