Investigation Summary
At 6:45 p.m. on April 16, 2008, Employee #1, a warehouse worker for Sunfresh USA Inc., sustained a serious occupational injury when the Nissan forklift (Serial Number 4309) tipped over on the loading dock while backing out of the storage area. The facility involved in accident is a wholesale agricultural product packer and distributor. Employee #1 had been working for the company for one day. Employee #1's fifth finger on his left hand got crashed and amputated between the frame of the forklift and the trash bin. Employee #1 was transported first to St. Johns Medical Center in Oxnard then to the Simi Valley Hospital where he underwent surgery.
