Investigation Summary
On February 28, 2008, Employee #1 was working on a cable line when the line slipped over one of the roller guides. To avoid damage to the cable by stopping the line, the employee climbed over a cage with a coworker and attempted to lift it back into position. His hand was drawn into the guide and his finger was partially amputated. Employee #1 was not hospitalized. Relevant programs were reviewed and the employer was issued two general citation and one serious citation for lack of implementation of an energy control procedure.
