Employee's Fingers Are Amputated by Trommel

At approximately 10:00 a.m. on March 13, 2008, Employee #1, who was employed for nine years by the City of Los Angeles, Bureau of Sanitation, Solid Resources Processing & Construction Division, was performing his daily duties as a maintenance laborer. The methane gas system was maintained at the facility where Employee #1 worked. Mulch and semicompost were also made at that green waste facility. Employee #1 was trying to clean around the drive wheel of a Morbark trommel (Equipment Number 40282), located in the composting and mulching facility. His hand was caught between the unguarded running metal drive wheel and trommel drum. Employee #1 was transported to Olive View Hospital in Sylmar, CA, where he was hospitalized and treated for two days. Three of Employee #1's fingers were apparently crushed, and they were amputated. Employee #1 had been working with or in close proximity to onsite employees, including a supervisor. Investigators interviewed Employee #1, his supervisor, and onsite employees. The investigation concluded that California Code of Regulations (CCR) Title 8 Section 4002(a) was violated, and the Division issued a serious accident-related citation to the employer.
