Employee Suffers Serious Burns in Chemical Fire

On February 28, 2008, Employee #1 was working at the "Hydro Blast Automatic Conveyor Parts Washer". The employee was standing beside the conveyor which carried baskets of parts into the parts washer. He had been observed earlier by the supervisor, standing at the conveyor near the entrance, and there was an open container sitting on top of the emergency stop button which was next to the conveyor entrance. The employee was using solvent in the container to wash parts before placing them into the baskets. The employee, apparently, knocked over the open container of solvent, and it spontaneously ignited. The employee's clothing caught fire, and he was taken to local hospital. Employee #1 was transferred to a burn center, where he was diagnosed with second and third degree burns to his right arm and was hospitalized for sixteen days and underwent skin graft surgery. The employer was cited for Serious Accident-related - T8 CCR 5549(a) Source of Ignition.
