Investigation Summary
At approximately 9:38 a.m., Employee #1, a 24 year-old male, with Leading Industries Inc., was seriously injured while adjusting a packing machine. Employee #1 was setting up and adjusting the index on a packing thermo former. Employee #1 adjusted the machine with his hand blocking a safety interlock and resulted in the crushing injury of his wrist and forearm. The site manufacturers plastic containers and was staffed by two employers, Tri State Staffing and Leading Industries who supplied and managed on-site employees. The Employers Injury and Illness Prevention Programs were reviewed and well as other documents. One regulatory citation and two serious citations, one accident-related for not implementing control of hazardous energy (lockout/tagout) procedures.
